- Please describe how you will use these funds. Who will be the beneficiaries of this grant?

- Have you received or applied for other grants for the purpose stated in this application? If so, please indicate current or potential grantors with amounts awarded or requested from each.

Please Note: There is an expectation that our grantees will report outcomes to our club either in writing or through a presentation at one of our meetings. We ask recipients to contact us within one year of receiving a grant to notify us of how you wish to make your report.
